Position Summary

The Hospice Social Worker provides emotional, psychosocial, and practical support to hospice patients and their families. Working as a member of the Interdisciplinary Team (IDT), the Social Worker helps patients and caregivers cope with the emotional, financial, and social challenges of end-of-life care. The role ensures that care aligns with the patient’s goals, cultural background, and family needs while complying with Medicare, state, and accrediting body standards.

Key Responsibilities

1. Patient and Family Support

– Conducts psychosocial assessments of patients and families upon admission and throughout care.
– Provides counseling to patients and families regarding grief, loss, coping, and adjustment.
– Supports families in navigating anticipatory grief and bereavement risk factors.
– Advocates for the emotional and psychosocial needs of patients and caregivers.

2. Care Planning and IDT Participation

– Develops individualized care plans based on psychosocial assessments.
– Collaborates with nurses, physicians, chaplains, and volunteers in IDT meetings.
– Provides input regarding patient/family needs, coping styles, and resources.
– Updates and revises care plans as patient/family needs evolve.

3. Resource Coordination

– Assists patients and families in accessing community resources.
– Provides education on advance directives, healthcare proxies, and legal/ethical issues.
– Connects families with bereavement services following the patient’s death.

4. Compliance and Documentation

– Completes psychosocial assessments, care plans, and progress notes in compliance with Medicare Conditions of Participation.
– Maintains accurate, timely, and confidential records.
– Adheres to HIPAA regulations and agency confidentiality policies.

5. Education and Advocacy

– Educates patients, caregivers, and community partners on hospice philosophy and resources.
– Provides staff training on psychosocial issues and cultural sensitivity.
– Advocates for patients’ rights, dignity, and access to care.

Qualifications

– Master’s Degree in Social Work (MSW) from an accredited program.
– Current state license (LSW, LISW, or equivalent).
– Minimum of 1–2 years of hospice, palliative care, or healthcare experience preferred.
– Knowledge of grief counseling, crisis intervention, and community resources.
– Strong communication, empathy, and collaboration skills.

Work Conditions

– Works primarily in patient homes, assisted living facilities, and nursing facilities.
– Travel required within the hospice service area.
– Flexible schedule, including availability for crisis intervention and IDT meetings.
