Ensuring Quality Hospice Care

Hospice care at ZCS is focused on enhancing the quality of life for individuals facing life-limiting illnesses. We offer services in a private home, hospital, or assisted living facility—whichever environment the patient feels most comfortable in.
Our care model is not just about treating symptoms, but about supporting the whole person: emotionally, physically, and spiritually.

Personalized Plan of Care

A collaborative care plan is created with input from the hospice nurse, healthcare providers, and the patient’s family. This plan is flexible and updated based on changes in the patient's condition or wishes.
A structured daily routine tailored to the patient's preferences
Medical appointments scheduled and coordinated by the team
Regular visits and wellness check-ins
Therapeutic activities for mental and physical stimulation
Home medical equipment like hospital beds or mobility aids
Personal safety plans, including fall prevention and medication safety
Nutrition planning, including meal assistance or specialized diets

Your Hospice Care Team

Our patients are never alone. They are surrounded by a compassionate and highly skilled team, each professional playing a unique and essential role in their well-being.
Hospice-Interdisciplinary-Team
Core Team Members:

Hospice Medical Director
Oversees all medical aspects of care and ensures treatment aligns with best practices and patient needs.

Attending Physician
Works directly with the medical director to direct all care decisions, supervise medications, and adjust treatment as needed.
03.
Registered Nurse (RN)
Monitors the patient’s condition, manages medications, and educates caregivers on symptom control and comfort measures. RNs are available on call 24/7.
04.
Hospice Aide
Offers essential help with personal hygiene, dressing, feeding, and light household tasks. They also offer a comforting presence and companionship.
05.
Social Worker
Provides counseling, assists with end-of-life planning, and connects families to financial and legal resources. Their goal is to support emotional and practical needs alike.
06.
Chaplain
Delivers spiritual and emotional care, whether religious or not. They help patients find peace and help families navigate difficult questions and transitions.
07.
Therapists (OT/PT/ST)
Occupational, physical, or speech therapists may be brought in to ease discomfort, improve communication, or boost daily functionality.
Additional Support Team:

Trained Volunteers
Provides companionship, helps with errands or light housekeeping, and offers caregivers much-needed breaks.

Dietitians
Ensures the patient’s nutritional needs are met with compassion and understanding, adapting to changes in appetite or ability.

Pharmacists
Reviews medications for safety, manages drug interactions, and assists with home delivery of prescriptions.

Grief Counselors
Available to support family members for up to 13 months after a loved one’s passing, helping navigate the grieving process.

Yes, hospice care can be provided at home. Our team is trained to deliver high-quality care wherever the patient is most comfortable—be it at home, in an assisted living facility, or hospital.
02. Can the plan be updated if my loved one’s condition change?
Absolutely. The plan of care is flexible and reviewed regularly. If there are changes in the patient's condition, the team will meet with the family to revise the plan accordingly.
03. How soon can hospice care begin after a referral?
In most cases, hospice services can begin within 24–48 hours after a referral and initial assessment, depending on location and urgency.
